Welcome to Leardi Family Dentistry. We are happy that you have chosen us to care for your dental needs. We have been treating patients in the Kennett Square area for over eleven years. He and his team of dental professionals are dedicated to bringing you the highest level of dentistry. We offer a wide variety of services ranging from family care dentistry to advanced aesthetic and rehabilitation procedures.

We strive to provide our patients with the individual attention and care they deserve. In order to fulfill this commitment to our patients, we have instituted some office policies:

  1. Our goal to to be ready to serve your needs at the time your appointment is scheduled, so we request that all of our patients be prompt for their appointments. We value your time as well as our own and one late patient tends to inconvenience all subsequent patients. If you are 15 minutes late, we will ask you to reschedule for another time.
  2. Payment is due at the time of service. All credit cards are accepted. We extend a 10% discount for cash, check, or debit cards and 5% for regular credit cards. We will file with your insurance company for services redered that day. Insurance will be billed as a courtesy, and insurance quotes and estimates are not a guarantee of payment.
  3. There is a 24 hour cancellation policy. It has become necessary to charge for missed appointments. If your schedule changes and you are unable to keep your appointment, please notify us as soon as possible. You can leave a message on our voice mail when we are not in the office. If you cancel with less than 24 hours of notice, it will be necessary to charge you for a missed appointment.
  4. Children under 18 should be accompanied by a parent during their appointments. If a parent cannot be present, we need a signed and dated note, giving us permission to perform treatment and noting any new medications or changes to medications.

At each appointment we would live you to arrive ten (10) minutes before your scheduled time. You will be asked to complete a Medical History and HIPPA form at your initial appointment. If you prefer, you can print these forms from our website, and bring in the completed form at you scheduled time. For any other forms
