To make your first visit as convenient as possible, we’ve made our New Patient and HIPPA Forms available to you online. Now you can download both forms to your computer and complete them at your convenience using the Adobe Reader software (available free of charge from Adobe.com). Just complete the interactive forms, print them out, and bring them to your first appointment. It’s that easy!
